Diabetes Advanced Practice Provider - Nurse Practitioner/Physician Assistant 

Good Samaritan is searching for an Advanced Practice Provider with a passion for diabetes education and care. 

This is a unique outpatient opportunity to join a Diabetes Care Team and be trained by a physician who has specialized in successful diabetes management for decades.

Future growth to include Endocrinology

Practice Highlights:

  • Manage patients with diabetes in a successful outpatient practice
  • Work closely with an experienced physician in a busy office
  • Support staff includes experienced RNs, LPNs, and receptionist
  • Case load averages 10-12 patients per day
  • Monday – Thursday 8am – 5pm

Requirements:

  • Passion for comprehensive diabetes care and education
  • Working knowledge of the medical management of diabetes & familiarity with CGM (Continuous Glucose Monitor) and insulin pumps
  • Candidates must have completed a Master’s degree from an accredited Physician Assistant or Nurse Practitioner program, be Board Certified and able to obtain an unrestricted Indiana license and DEA
  • Desire to work with a team focused on the whole patient from lifestyle to medication management
  • EPIC experience preferred
